Club Forms

Listed below are all of the forms and USA Water Polo Membership that are required for each session.  Please download the forms and send them with your dues prior to the start of each session.

Registration Form:

Each athlete must also have a club registration form on file on their very first day of participation whether it is a trial membership or full session membership.  Please mail you medical form with your dues prior to the start of the each session or bring it with you if you at doing a trial membership.

Code of Conduct:

All participating athletics must also have on file a signed SWPA Code of Conduct form.  Like the medical from in order to participate this form have to be mailed in with your club dues prior to the start of each session.

Membership with USA Water Polo - Mandatory to Play

USWP Verification is a must!  All players must be a registered member of USA Water Polo and have their membership card with them.  If you have yet to register for the 2009 year please visit either;

Trial Membership Forms:

(For first time athletes only) This allows you the opportunity to come out a try water polo for a week.  In order to do this please download and complete the "Trial Membership" and Registration forms and bring those with you along with a check payable to SWPA for $20.00.  If your athlete decides to join SWPA after the week trial period the trial membership will be applied to the total session's dues and your child must become a member if USA Water Polo and pay the remaining dues during your next visit.
